Built in 1918, this Dutch Colonial Revival-style house at 412 N. Carroll Street in Madison, Wisconsin, features a red brick base with a stucco-clad gables, gambrels, and dormers, a front gambrel roof with a side gable and multiple shed dormers, a front porch with brick columns, railings, and arched openings, a front door with a fanlight transom and sidelights, one-over-one replacement windows, a fanlight attic window, and modillions at the eaves of the porch roof.

The house is a contributing structure in the Mansion Hill Historic District, listed on the National Register of Historic Places in 1997.
