脳内のドパミン２（D2）受容体を遮断することで、ドーパミン神経系の機能亢進により起こる陽性症状をおさえる。また、セロトニン２（5-HT2）受容体を遮断することで、ドーパミン神経系の働きがよくなり、陰性症状が改善する。このような作用メカニズムからから、セロトニン・ドーパミン拮抗薬（ＳＤＡ：Serotonin-Dopamine Antagonist）または5-HT2/D2拮抗薬などと呼ばれる。そのほかにも、アドレナリンやヒスタミンなどいろいろな受容体に作用することから、多受容体作動薬（MARTA：Multiacting Receptor Targeted Antipsychotic）に分類されることもある。
