Printed and handwritten on image: Application and Affidavit for Placer Mine. "I G.W. Carmack of Forty-Mile" hereby apply, under the Dominion Land Mining Regulations for a mining location in "a Creek known as Bonanza Creek flowing into Klondike River. Discovery claim on Bonanza Creek" for the purpose of mining for "gold"; that I have discovered therein a deposit of "gold"; that I am to the best of my knowledge and belief, the first discoverer of the said deposit; that I am unaware that the land is other than vacant Dominion land; that I did on the "17th" day of "August 1896" mark out on the ground in accordance in every particular with the provisions of the said Mining Regulations, the location for which I make this application ... PH Coll 692.13

George W. Carmack was a gold miner in the Yukon Territory and is credited for discovering gold on Rabbit Creek, (later Bonanza Creek), which launched the Klondike Gold Rush in 1896. Carmack later left his family and the Yukon and remarried in Olympia, Washington in 1900.
