Происходит из часовни Зверина монастыря в Великом Новгороде. С 1914 года находилась в Новгородском епархиальном древлехранилище, затем – в Новгородском музее, с 1945 в ГЦХРМ

Реставрирована в 1948–1949 годах в ГЦХРМ В.В. Филатовым и Ф.А. Модоровым
