whichextend down to the urethra. No drain should be introduced. Primary union isobtained along with an excellent passage for the urine. The bladder should be keptempty by syphon drainage, and irrigation should be carried out. 172. Exposure of the Membranous and Prostatic Parts of the Urethra (Fig.127 or 128). This operation also gives access to the prostate, vesicuke seminales, thevasa deferentia, and the floor of the bladder. The external incision must be here anextensive one, so that the median incision is no longer suitable. A purely lateralincision, as formerly employed, divided the branches of the internal pudic artery andnerve passing towards the middle line, viz. posteriorly, the inferior haemorrhoidalvessels and nerves, anteriorly, the superficial perineal vessels and nerves and the arteryto the bulb. Although the sacral resections have shown that unilateral division ofthese nerves does not necessarily lead to permanent motor disturbances, nevertheless IS 274 OPERATIVE SURGERY
