Before proceeding to the second step of the operation a flap should be marked out on the outer sideof the pectoral region; to do this the knife divides the skin above the middle of the first incision, i.e., onthe outer edge of the wound, on a line drawn first at right angles to said incision and gradually sweepinground until it becomes parallel to it and terminates at a point a little below the level of the lower marginof the wound. This flap is intended to be turned into the lower portion of the wound. In case thereis infection of this cervical region, an additional incision should be made from the middle of the upperhalf of the median incision along the posterior border of the sterno-cleido-mastoid muscle to expose theposterior cervical (subclavian) triangle. This incision, if necessary, is not to be made until a later stage of the operation.
