I'm glad you received a reply rather quickly at the OTRS noticeboard. That declaration of consent is very easy to send in (it's basically a copy-paste job, but using your specific details). The subject of the letter can be "Granting permission to use an image", or "Releasing an image under a free license", for example. Make sure you include both links (the link created by Commons, and the one on your website), in your letter. You won't have any problems since I see that you have an info@graysonhoffman.com email account, which is associated with your website, www.graysonhoffman.com That is not always the case.
Please post a message at the nomination for deletion page stating that you have sent in the formal declaration of consent once you have done so, here: Commons:Deletion requests/File:Grayson Hoffman with his wife Jamie.jpg. That will halt the deletion. Then it's just a matter of waiting for clearance from the OTRS team, which can take weeks, but in the meantime keep the picture on the Wikipedia page.
Please don't take this situation personally. I have requested the deletion of around a hundred pictures just over the past two weeks, of which around 95% get deleted because I'm right. Even in your case I was right because the proper procedure was not followed, but since you do own the picture, I'm doing my best to help. I just try to enforce the rules. Thanks for your cooperation. Sorry for all the confusion, this is my first time uploading the photo and I obviously don't know what I'm doing, event don't know how to write a reply. Thank you for your help!

Hi again, Odmitrieva. Thanks for being so courteous and understanding. In a perfect world what you did would have been fine, but unfortunately tons of people upload pictures here claiming "own work" when in reality they often find those pictures on the Internet or in magazines, so of course that causes copyright violations. Therefore, proof is required, especially when a picture looks too professional or can be found on the Internet.
I have a question: how was the picture taken? If you somehow set up a camera to take the picture, then that's fine, but if someone else took the picture, you will run into a problem because that person would own the picture. If you set up a camera on a tripod (or whatever) to take the picture (or you did something along those lines), then please say it in your declaration of consent. If Grayson was in charge of the process, then state that Grayson Hoffman is the creator and sole owner of the exclusive copyright of the photograph. If it was Jamie, then state that Jamie Hoffman is the creator and sole owner of the exclusive copyright of the photograph. If someone else took the picture, then your declaration of consent won't work. By the way, here it is again:

That's all you have to send in, but as I mentioned, please add how the picture was taken if Grayson or Jamie took it because the logical assumption is that someone else took it. If you can give me that information here, I will customize the letter for you so that all you would have to do is copy what I write here, and paste it into a new letter, to save you time. You apparently forgot to log in before replying! Anyway, many thanks. It seems as if you were both involved in the creation of the image, which is fine. I will write a customized declaration of consent right here but please give me a few hours (maybe less). Obviously you can change anything before you send it in if so you wish, but the main statements must be included. I'll be back. Thanks again for your kindness.
