Built in 1878, this Italianate-style house at 404 North Carroll Street in Madison, Wisconsin, was constructed as an income-generating rental property for John Lewis. The house is clad in wooden clapboard with a front gable roof, a fanlight attic window, one-over-one replacement windows, decorative trim surrounds at the windows, a two-story bay window on the side with decorative brackets and panels, a stone base, a front vestibule, and a stone-clad basement with a terrace on the roof that houses a garage.
The house is a contributing structure in the Mansion Hill Historic District, listed on the National Register of Historic Places in 1997.
