Description
中文（繁體）：原嘉義縣東石鄉洲仔村慈安宮（現擴建為臥龍港代天府）吳府千歲於鄉里間救人無數，神威傳播。公元1956年取陳氏為乩生後大發神威，專事抓妖、治病、消災、解厄等科儀，時因村中子弟大量外出發展，東石鄉洲仔村人氏陳榮場攜眷北上至臺北縣板橋市，自臥龍港代天府（時之慈安宮）奉請中壇大元帥、二元帥、三元帥赴自宅中奉祀。公元1977年7月19日，地方仕紳見香火鼎盛徵得陳氏同意，擇地遷至陳政源處，擇良辰吉日興建小祠，名曰：慈安宮，日後支柱日增。公元1981年，因信徒眾多致使原址不敷使用，遂擴建小廟於今新北市板橋區懷德街174巷18弄18號址，廟內分有三殿：前殿供奉中壇元帥、五府千歲等眾神，中殿供奉天上聖母、觀世音菩薩等眾神，後殿供奉城隍爺、地藏王菩薩等眾神。
