English: Page from Archive of St Maria Magdalena Parish Church in Cieszyn showing birth of Bogusław Jan Sylwester Kusionowicz, son from first marriage of Andrzej Kusionowicz, on 19 August 1892 including indication of later change of surname to Grodyński.

Copy of scanned image courtesy of Michael Morys-Twarowski with note: "Registers of baptism for Oberstadt from years 1892-1894 (Vol. 22, Obervorstadt, page 140): On 19 August 1892 in Wyższa Brama 233 was born Bogusław Jan Sylwester, son of Andrzej Kusionowicz and Anna nee Wesnicky Palasek. His godparents were Fryderyk Wesnitzky, owner of house in Kremsier, and Aniela Kusionowicz, wife of a notar in Milówek (probably Milówka). There is also a note that by decision of Rząd Krajowy w Opawie (Government in Opawa) from 19 December 1906 (no. Z 35805) the surname was changed to Grodyński and this decision was valid from 7 September 1906. [Prad # Z 266.81]"
