Latina: Banderium Cunradi Albi, ducis Oleschniczensis Zlesie, quod presencialiter ipsemet Cunradus dux de gentibus propriis ducebat; sub quo erant milites sui proprii de ducatu Wratislauiensi et de Slesia. Et ipse dux Cunradus fuit captus, et tam banderio quam omnibus fortunis suis spoliatus, liberalitate autem Wladislai Polonie regis in libertatem restitutus.
Polski: Chorągiew Konrada Białego, księcia oleśnickiego ze Śląska, pod którą stanął osobiście sam książę Konrad wraz ze swoimi ludźmi. Byli więc razem z nim jego właśni rycerze z księstwa wrocławskiego i ze Śląska.
Chorągiew miała długość dwóch łokci i ćwierć (ok. 140 cm), a szerokość dwóch łokci bez ćwierci. (ok. 110 cm)
