English: This is described as a "Clock Generator for ATI RS480 Chipset" and it was made by Cypress Semiconductor Corporation around 2005.
Since it is built for the RS480 chipset I know this is from the motherboard that housed the Athlon 64 4400+ I imaged previously. There appears to be writing in the top right corner, but I was unable to make it out at higher magnifications. When cracking this chip's package open I was worried I would crack the die because of how long and thin the package was. To minimize this risk I took extra time to thoroughly heat up the package before compressing it in my vice.
