«Прямоугольное двухстороннее полотнище с отношением ширины к длине 2:3, воспроизводящее фигуры герба Середняковского сельского поселения, выполненные в жёлтом, зелёном, белом и синем цвете».

Геральдическое описание герба: «В рассеченном золотом и лазоревом поле на серебряной и лазоревой волнистой оконечности, дважды просеченной, вверху бегущими вправо волнами, а внизу волнисто — сосна с зелёной хвоей, переменяющая цвет ствола и ветвей с зелени на золото».
