From source (in German): Die zur Krankenhaus-Einweisung über Semmelweis gefertigte "Krankheits-Vorgeschichte" von János Bókai

My translation: The medical examination report [Krankheits-Vor-geschichte is literally disease-pre-history] on Semmelweis by János Bókai used for hospital admission.

Note: At his own request, Semmelweis was examined by János Bókai (likely this examination report). János Balassa later wrote a referral to an insane asylum which according to regulations should be signed by three physicians; they were János Wagner and János Bókai (Carter and Carter 2005:74-75) (unlikely this document). I believe there is an ambiguity in the source text caption, because János Bókai at the time of the examination probably was unaware that Semmelweis later would be referred to an insane asylum.
