Odznaczenie ustanowione uchwałą Rady Regencyjnej Unii Polskich Ugrupowań Monarchistycznych nr 15/2010 z 11 listopada 2010 roku. Medal jest odznaczeniem jednostopniowym nadawanym przez regenta długoletnim członkom UPUM za okazaną wierność Unii oraz innym osobom za aktywną działalność w rozbudzaniu świadomości idei monarchistycznej w społeczeństwie. Oznakę odznaczenia stanowi krążek barwy złotej o średnicy 35,00 mm. Na awersie umieszczony jest herb Grzymała na płaszczu heraldycznym, nakryty mitrą książęcą, a powyżej na obwodzie napis w dwóch rzędach "Regent i Wielki Książę UPUM / Aleksander Podolski". Na rewersie w wieńcu laurowo-dębowym, umieszczone trzy gwiazdki, kotwica i sentencja "Per Aspera ad Astra". Całość zawieszona jest na wstążce o szerokości 40,00 mm barwy ciemnoniebieskiej z dwoma żółtymi paskami.

Awers "Medalu Regenta i Wielkiego Księcia UPUM Aleksandra Podolskiego" projektował Norbert Wójtowicz, rewers projektował Arkadiusz Wł. Drajczyk.
