This is believed to be one of two surviving models by Mochi, painted in imitation of a relief in pietre dure. The relief was commissioned by Cosimo II to fulfil a vow he made during illness, and was intended as the centrepiece of a gold altar-frontal for the altar of St Charles Borromeo in the Cathedral of Milan. The Grand Duke kneels before an altar, with the cathedral of Florence visible through the window behind.

Cosimo died in 1621 and after completion it was retained by his sucessor in the Guardaroba Medicea. In the late 18th century the golden parts of the frontal were melted down and the central relief reset in its present frame.
