The externaljugular vein is sometimes opened in cases of apoplexy in adults,and in children if the veins at the elbow are hidden by fat. Thepatient, if a child, being laid in the nurses lap, with its head to-wards the surgeon, the latter puts his left thumb on the vein a littleabove the clavicle, and then opens it with a lancet at a pointmarked in the figure, cutting towards the thumb, and in a directiondownwards and inwards, so that the incision may cross the fibresof the platysma. The wound should be closed by lint and plaster.Instead of the thumb, a compress may be placed on the vein, and besecured by a bandage carried under the axilla of the opposite side.If an adult, the patient may be placed in the sitting posture, if pre-ferred.
