Cervicalglands palpable. Heart impulse heaving and dif-fuse, and seen in 4th interspace. Sounds regularand slightly rapid. First sound replaced by sys-tolic murmur, transmitted to axilla, precordia andback. No definite thrill. P2 greater than A2. Re-duplicated and accentuated. Arterial walls not palp-able. Brachials not tortuous. Blood-pressure 140/70.Knee jerks present and equal. Laboratory Findings: Urine negative. Wasser-mann negative. the aortic arch and a prominence in the lowerpart of the ascending portion which probablyrepresents a localized dilatation. The heartis very long and lies transversely in the chestas a result of a high diaphragm. The aur-icles are not enlarged, but the curve of theleft ventricle is very prominent. The dis-tance across the base B is not outsidenormal limits, and it may be inferred thatthe left ventricle is enlarged much more thanthe right. CARDIO-RENAL DISEASE A patient with damaged kidneys who hasmaintained a high blood-pressure for a longtime must of necessity possess a large hyper-trophied heart if he is to keep his bodilymachine operating. If the hypertrophy is notsufficient to meet the heavy demands of ahigh blood-pressure, he suffers from variouscardiac symptoms. The hypertrophy isusually pretty well distributed between thetwo ventricles, and as a result such heartsbecome enormous in size. Figures 17 and 18show fairly typical cases. The aortic knobsare prominent, perhaps as a result of arterio-sclerosis, which is often present in cardio-renal disease. Although the curves repre-senting the auricles are not at all prominent,the distance B is outside normal limitsand this seems to indicate the presence of adefinite hypertrophy of the right ventricle.The increased length of the heart and theprominence of the curve of the
