Need help with English Wikipedia Unblock Ticket Request System

Hi Just Chilling,

In 6 Nov 2017, you kindly unblocked my account from my site's IP range:

2017-11-06T14:34:12 Just Chilling (talk | contribs) changed group membership for Cmglee from extended confirmed user to IP block exempt (temporary, until 2018-11-06T14:34:12) and extended confirmed user (Trusted user regularly affected by IP blocks)

It has expired and I'm blocked again (which is why I'm messaging you at Commons).

When submitting an unblock request at https://utrs.wmflabs.org/index.php as before, I get this error

There were errors processing your unblock appeal: 
We need to know which administrator placed your block

but there was no field to state that User:SQL blocked this range.

There's also this line at the top of the page:

Notice: Undefined index: by in /usr/utrs/production/public_html/src/appealObject.php on line 226

Can you please help me?
